If you are not automatically prompted to complete the application, please download it here. You will also need to complete the Resident Profile Form. Please send these documents to 100midtown@aacre.com or fax to 404.961.7139.
• Step 3: Download the Authorization to Release Information Form and fax or e-mail this information to 100 Midtown.
